Reset Password

 

Current password for scheduler: Enter in your password here. The password for a provider can be changed only if the password for the current location administrator is confirmed. This is done to ensure that someone other than the location administrator resets the password.

 

New password:The password has to be a minimum of 12 characters long, with one or more upper case characters, lower case character, and digit.

Confirm password: Re-confirm the new password.
